The Squirrel Hill Family Wellness Center offers a variety of appointments to suit the needs of your entire family. Please call us for more information on any of the following appointment types.
INITIAL OFFICE VISIT (90 MINUTES)
This initial appointment is to become a new patient in our practice.
Please note: There may be additional fees for supplements, remedies and medications.
RETURNING PATIENT VISIT (60 MINUTES)
After your initial office visit, your first follow-up will be 60 minutes. This also applies to already established patients who have not been seen in our office for 2 or more years or for patients with extensive health issues requiring a longer appointment.
FOLLOW-UP VISIT (30 MINUTES)
General follow-up visits are 30 minutes long and can be in the office or over the phone.
TESTING, SUPPLEMENTS, REMEDIES, & MEDICATIONS
These are an additional cost.
PAYMENT: Payment is expected at the time of your appointment. Payment may be made by personal check, cash or credit card (Mastercard, Visa, Discover). Payments for phone consultations must be made by credit card at the conclusion of the consultation.
INSURANCE: If you have medical insurance, we will provide you with a SUPERBILL that can be attached to your claim form. We require that you initially make payment to us and subsequently seek reimbursement from your insurance company. Filing your own claim saves you the added expense and time required for us to do so. As required by law, we submit claims for Medicare recipients which will be directly reimbursed to them.
APPOINTMENTS: We follow these procedures to keep the lines of communication open and to minimize unnecessary charges for missed appointments.
New Patients should complete the questionnaire and bring it to the first visit along with any blood work or diagnostic test results. Although your first appointment is 90 minutes, please plan for an additional 30-45 minutes in the office to allow for appointments running late and for checking out after your appointment.
You will receive a confirming call 1 to 2 business days before your appointment. If we have not been able to confirm your appointment with you and have not received your questionnaire, your appointment may be given to another patient and you will be charged for the appointment.
We are a fragrance-free office. Out of respect to our chemically sensitive staff and patients, please refrain from wearing any fragrances the day of your appointment.
